Why operator logs do not carry pattern recognition
Control rooms are heavily instrumented and heavily documented. Every parameter is trended, every alarm recorded, and the log and shift turnover are formal processes.
What none of that captures is the operator's attention. The log records that a value was within limits; it does not record that an operator had been watching it for twenty minutes because the spread looked wrong. That anticipation is the whole skill, and it is invisible to every system in the room.
